HAVAFREEPOKER MEDIA RELEASE, RULES AND WAIVER FORM
I hereby grant HavaFreePoker ("the Organization") permission to capture and use my likeness, image, voice, and performance in photographs, video recordings, audio recordings, and any other media ("Content") for any purpose related to the Organization's mission, including but not limited to: Educational materials, Promotional content, Social media posts, Website content, Print publications, Broadcast media.
By signing this form, I acknowledge and agree to the following:
- Ownership: All Content featuring me becomes the sole property of the Organization.
- Usage Rights: I grant the Organization unlimited, worldwide rights to use, reproduce, distribute, edit, exhibit, and publish the Content in any medium, without restriction or compensation.
- Waiver of Rights: I waive any right to:
- Inspect or approve the finished Content
- Receive royalties or other compensation for the use of the Content
- Pursue any claims related to the use of the Content
- Release of Liability: I release the Organization, its employees, volunteers, contractors, and affiliates from any claims, damages, or liabilities arising from the use of the Content.
- Duration: This release is valid for perpetuity from the date signed, unless revoked in writing.
- No Expectation of Privacy: I understand that my participation may be in public settings with no expectation of privacy.
- Representation: I affirm that I am over 18 years of age and competent to sign this release.
- Gambling Prohibition: I agree that no gambling will take place during the creation of the Content or in relation to my participation with the Organization. I further agree not to engage in any form of gambling in connection with this agreement or while representing the Organization in any capacity.
By signing below, I confirm that I have read, understood, and agree to the terms of this Media Release and Waiver Form.
